Folders are printed in blue ink on 90 lb.
white index stock and scored so that
they can be easily folded in half.
Overall size when folded is 3 1/2" x 2 1/4"
and will easily fit into a wallet or in one
of our medical card holders.
The cards have space for listing up to 15 medications.
There is also a space to list known allergies.
Front of folder has space for
Pharmacy name,address,
phone number and
other copy as space permits.
